Each Case Discussion webinar will be broadcasted live via the ZOOM Webinar platform for approximately 40-60 minutes.

Title: Cardiac Arrest and Prolonged Apnea in a Patient with Infantile-Onset Epilepsy: Epileptic or Non-Epileptic?
Description:
Summary of the case: A 3.5-year-old girl with developmental delay and epilepsy presented with recurrent paroxysmal events, autonomic dysfunction, and severe cardiorespiratory instability requiring ICD implantation. Her events included staring episodes, tonic posturing, hypotonia, cyanosis, generalized tonic–clonic seizures, and episodic apnea, with gait impairment and speech slowing developing during follow-up. EEG findings remained non-specific, and some clinically concerning events lacked clear electrographic correlation. A significant family history, including consanguinity and an affected sibling with a similar clinical history, raises suspicion for an underlying genetic disorder. Ongoing investigations are focused on distinguishing epileptic, autonomic, and movement-related phenomena.
Description: This EPNS Case Discussion will explore the diagnostic journey of a young child presenting with developmental delay, epilepsy, ataxia, autonomic dysfunction, and severe cardiorespiratory instability requiring cardiac device implantation. The case highlights the complexity of distinguishing epileptic, autonomic, and movement-related phenomena in a child with a suspected genetic etiology, reflecting the diagnostic uncertainty often encountered in paediatric neurology.
We will discuss:
• Diagnostic challenges in a child with developmental delay, epilepsy, ataxia, and autonomic dysfunction
• The complexity of distinguishing seizures from autonomic and paroxysmal non-epileptic events
• The role of multimodal investigations, including neurophysiology, sleep studies, genetics, cardiology, and respiratory assessment
• The importance of multidisciplinary collaboration in reaching a working diagnosis and guiding management
